Dive Forms

As they say, the job’s not done ’till the paperwork’s finished. Now we all know watersports are inherently dangerous. For this reason the Club needs to operate with a strict policy regarding the handling of release forms.

Scuba Diving Release

Bimini Big Game Club Dive Center needs to collect two forms – the LogIn form and the Release Form (which varies based on the dive activity selected).

The LogIn form activates your planned dive trip to the Bimini Big Game Club. This is where we collect your SCUBA Certification information (training agency and certification number) and determine what SCUBA gear you will be bringing, and what your equipment rental needs will be.

The Release Form establishes your acknowledgement that snorkeling and/or diving is inherently dangerous, and you are releasing the Club and its affiliates from any liability relating to your use and participation in any or all of the Club’s snorkeling and/or diving activities. Our release form is the industry standard form used worldwide by PADI (the Professional Association of Diving Instruction). We trust you understand that we are not at liberty to accept any modifications of any kind to the form.

Who Must Fill Out the Forms?

Anyone who is going aboard any Club dive boat, for any reason, must be accounted for on separate Sign In and a Release forms. This includes

  • Adults (18 & older) – Each adult guest must fill out and sign their own individual waiver
  • Minors (17 & under) planning on SCUBA Diving – Each minor guest must fill out and sign their own individual waiver and the waiver must be co-signed by a parent or legal guardian.
  • Minors not engaged in SCUBA diving – A maximum of 2 children may be added to a parent/guardian’s primary Waiver form.

Fishing/Watersports Release Form

If you’ve booked through the Club an offshore fishing charter, a guided backcountry fishing trip, or any other watersport rental items through the Club, we collect just a Release Form. This particular form establishes your acknowledgement that fishing, especially from a boat, is inherently dangerous, as is boating or other activities occurring on the water and you are releasing the Club and its affiliates from any liability relating to your participation in these activities. We trust you understand that we are not at liberty to accept any modifications of any kind to the form.

Submitting the Forms

In filing out your form, please use your Full Name (as it appears on your passport) and your Date Of Birth (aka DOB). This is the only information we need. Please note that our forms use the North American format for dates – month/day/year. Once you have filled out all of the appropriate form fields, please print out the form, sign, and date it. Print out 2 copies if you wish to keep one for your records.

Once your forms are completed, dated and signed, FAX or email them to Dive@biggameclubbimini.com. The other option of course is to simply bring the completed and signed forms with you and give them to the Outfitter staff upon your arrival. Regardless, you’re one step ahead to a great and memorable vacation with this paperwork behind you!

Please Note : The resort pool will be under renovation from April 6 – June 5, 2015. We apologize for the inconvenience. And although the resort is located on the water, an alternate swimming option is the beach, just a 3 minute walk across the island from the resort entrance.
